Lost or Stolen Passport Replacement

Important: We can only process passports & visas for US residents. If you live outside the United States we cannot help you obtain a visa for travel.

In order for us to process your US passport for you, you must first go to a local acceptance location which will either be a special post office or county clerk's office. Make sure you take sufficient identification, such as a driver's license with you. You will need to show the items listed below, which will then be put into a government sealed envelope. Make sure you get the envelope back from the clerk, and do not open it.

Lost Passport Replacement Requirements

1. DSP-11 brown passport application (print out and fill in blanks)
2. DSP-64 gray lost/stolen form (print out and fill in blanks)
3. Original birth certificate or other proof of U.S. citizenship, or original naturalization certificate, or a previous passport.
4. Two identical passport photographs.
5. Valid Picture Driver's License issued more than 6 months ago. If your Driver's License has been issued or renewed in the last 6 months, you will need to provide at least 3 personal documents.
6. Release letter (print out and fill in blanks)
   1. Both parents must appear with the child at the acceptance agent when obtaining the sealed envelope. If both parents cannot be present then a Written Statement of Consent must be provided to the acceptant agent.
7. 1 check made out to Department of State for $115.00 for adults and $100.00 for children and another check for $30.00 for the post office/county clerk's fee. No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.
8. Lost/Stolen Statement (print out and fill in blanks)
9. If you need a passport within 10 business days, then you will also need proof of departure, which will be ONE of the following:
   1. copy of your airline tickets or
   2. travel itinerary or
   3. company letter of expedite (click here for a sample)
10. Please fill out the order form and print a copy to send to us inside your Federal Express Priority Overnight, also in the Federal Express should be a Check for $99.00 or $149.00 (same day) for our service fee plus return delivery fees (see order form). If you need a same day service please call our office as we will need to fax a different release letter to you.

Before you go to your local acceptance location (Either a special post office or county clerk's office) please call us at 713-629-8330 so we can go over your information with you. We will make sure everything is in order. After you appear at your local acceptance agent you will then Federal Express your sealed envelope, order form and payment to us at:
