Same Day Instructions for Passport Name Change

Important: We can only process passports & visas for US residents. If you live outside the United States we cannot help you obtain a visa for travel.

If your passport is going to expire within 1 year you should follow the passport renewal instructions.

Follow these instructions if you are leaving the country within 4 business days.

Requirements for Same Day Passport Name Change

In order for us to process your passport, we must receive the following information from you:

1. DSP-19 green amendment form (print out and fill in blanks)
2. Your passport, which must still be valid.
3. Proof of your name change, such as: (originals only)
   1. marriage license or
   2. divorce decree or
   3. court order
4. 2 copies of Letter of Authorization (print out and fill in blanks)
5. Government fee - $60.00 No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.
6. You must have proof of departure, which will be ONE of the following:
   1. 2 copies of airline tickets or
   2. 2 copies of travel itinerary or
   3. 2 copies of company letter of expedite (click here for sample)
7. You must fax a copy of the Instructions for Same/Next Day Service of Passports to us so we know that you are sending a passport to us.We then reserve a space for your passport application with the passport office. Please print and fill out the Instructions for Same/Next Day Service of Passports and include a copy inside your FedEx Priority Overnight envelope.
