Passport Name Change

Important: We can only process passports & visas for US residents. If you live outside the United States we cannot help you obtain a visa for travel.

If your US passport is going to expire within 1 year you should follow the passport renewal instructions.

Passport Name Change Requirements

In order for us to process your US passport, we must receive the following information from you:

1. DSP-19 green amendment form (print out and fill in blanks)
2. Your passport, which must still be valid.
3. Proof of your name change, such as: (originals only)
   1. marriage license or
   2. divorce decree or
   3. court order
4. Release letter (print out and fill in blanks)
5. Government fee - $60.00 No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check. You can write one check for the governments fee, our service fee and return delivery by Federal Express. (For the correct amount please see the order form.)
6. If you need a passport within 10 business days, then you will also need proof of departure, which will be ONE of the following:
   1. copy of your airline tickets or
   2. travel itinerary or
   3. company letter of expedite (click here for sample)
7. Please fill out the order form and print a copy to send to us inside your Federal Express Priority Overnight. If you need a same day service please call our office as we will need to fax a different release letter to you.
