Same Day Instructions for Second Limited Passport

Important: We can only process passports & visas for US residents. If you live outside the United States we cannot help you obtain a visa for travel.

Follow the instructions below if you have a valid passport and require a second valid passport because you are a frequent traveler or travel to countries that are in conflict with each other and if you are leaving the country within 4 business days.

Requirements for Same Day Second Limited Passport

1. DSP 82 Red application form (print out and fill in blanks)
2. Your passport, which must still have at least 2 years validity.
3. 2 copies of Letter of Authorization (must print 2 copies out)
4. Government Fee - $115.00 No temporary checks will be accepted by the passport office. You can use a money order in place of a temporary check.
5. Two (2) passport photos
6. Letter from individual stating why a 2nd limited passport is required. Letter must state " If either passport is lost / stolen I will report it to the nearest US Embassy immediately." Click here for a sample letter.
7. If you are leaving the country within 10 business days, then you will also need proof of departure, which will be ONE of the following:
   1. two copies of airline tickets or
   2. two copies of travel itinerary or
   3. two copies of company letter of expedite (click here for a sample)
8. Please fill out the Instructions for Same/Next Day Service of Passports and print a copy to send to us inside your Federal Express Priority Overnight.
9. You must fax a copy of the Instructions for Same/Next Day Service of Passports to us so we know that you are sending a passport to us. We then reserve a space for your passport application with the passport office. Please print and fill out the Instructions for Same/Next Day Service of Passports and include a copy inside your FedEx Priority Overnight envelope.
